Description:
The OHSU hepato-pancreato-biliary fellowship program is designed to draw from the multidisciplinary strengths of faculty members in abdominal organ transplant/hepato-pancreato-biliary surgery, surgical oncology, and general surgery. The fellowship is aimed to complete the specialty training of well-trained general surgeons. The program is two years in duration with a new fellow matriculating yearly. The fellows alternate between the Surgical Oncology and Transplant/HPB services. The program is clinically focused and includes the fundamentals of hepato-pancreato-biliary surgery, liver transplantation and the evaluation of benign and malignant HPB conditions as well as liver failure patients.
While the fellowship focuses on learning the operative management, clinical practices and treatment planning related to HPB, several important components of the fellowship warrant further mention. First, an important goal of the fellowship is to develop clinical research experience. At minimum fellows will produce 2 projects annually with a faculty mentor(s) for presentation at national or international meetings followed by manuscript publication. Second, fellows are expected to grow into leadership roles as they develop into confident faculty. Finally, participation in didactic instruction for residents and students makes up another element of the fellow role at OHSU, as well as training surgical residents in the operating room.
Graduates will be expected to finish the program with substantial technical expertise in liver resection, complex biliary reconstruction, pancreas resection, liver transplantation, organ procurement, and minimally invasive liver and pancreatic surgery and techniques of liver tumor ablation. Robotic curriculae are currently being developed and fellows are participating in robotic cases with formal recognition and future certification. Trainees will be thoroughly trained in the evaluation and clinical decision making for patients with benign liver masses, primary liver, bile duct and gall bladder cancer, management of metastatic cancer to the liver, complex biliary diseases that need reconstruction such as bile duct injuries sustained during gall bladder surgery and biliary strictures after liver transplantation, straight forward liver transplantation, abdominal organ procurement, and neoplastic and non-neoplastic diseases of the pancreas and duodenum. In addition, experience in hepatic artery infusion techniques will be a part of the fellowship curriculum. While the program is not aimed to train liver transplant surgeons, the trainee will benefit from broad exposure to organ retrieval, recipient hepatectomy and liver transplantation. Trainees will be expected to develop independent clinical and translational research skills. Under faculty guidance graduates will be expected to design and complete a research project, with presentation at local, national and international HPB conferences, and publication of their work.
Ultimately the program is designed to produce superb technical surgeons with a thorough understanding of the clinical and translational aspects of HPB disease. The OHSU HPB fellowship aims to provide a structured mentorship environment tailored to the strengths and needs of each fellow. Ultimately, we expect the fellows to graduate the program as confident leaders in HPB surgery.

Clinical Outpatient Experience
The HPB fellow will attend at a mininum 2 half-day clinics where new and followup evaluations are done on HPB and liver transplant patients with a broad spectrum of diseases. There will also be inpatient consultations from other referring services that will span the spectrum of HPB diseases. The HPB fellow will learn independent clinical decision making regarding the work up and care of as well as the appropriate operative conduct for these patients, and will have the opportunity to follow his or her patients during the span of the fellowship.
Research/Scholarship Resources
Dr. Susan Orloff has an NIH-R01 - funded 500 square foot laboratory with PCR machines, hoods, cell culture, -80 freezers, and has access to a 5,000 square foot facility with all of the core facilities that include microaarray and genomic and proteomic high through put analyses. This provides a platform for basic and transplantational research should the fellow have the interest and experience in this arena. In addition, the Divisions of Abdominal Organ Transplantation and Surgical Oncology have long-standing databases that include the liver transplant recipients, and all of the HPB patients that have undergone surgery. In addition, our groups are part of a Multidisciplinary Liver Tumor Board that meets 2 hours per week and discusses on average, 15 cases per week and the HPB fellow presents on average, 2-4 cases per week. The membership on this Tumor Board includes Hepatologists, Liver Transplant/Hepatobiliary Surgeons, Surgical Oncologists, Medical Oncologists, Interventional and Body Imaging Radiologists, Radiation Oncologists, and access to a Pathologist. A real-time database and tumor registry supports this tumor board. The fellow will also attend and present cases to the weekly Mutidisciplinary Pancreatic Tumor Conference, and the Multidisciplinary GI Oncology Conference, each of which has an additional broad spectrum of complex and interesting cases.
In addition to the above, our Division Faculty are members of the Knight Cancer Institute that has a plethora of research opportunities and resources, which includes access to a biostatistician and institutional funding. The vast array of cases and resources affords the fellow to pursue a project(s) in any area encompassed by the field of HPB and Liver Transplantation.
The Department of Surgery has a Grants and Contracts Administrator who devotes her full time to assisting and administering all of the grants - both institutional, local, and national that stem from departmental research.

Dr. Rocha is the Hedinger Chair and Chief of the Division of Surgical Oncology at Oregon Health and Science University (OHSU) in Portland, OR. He also serves as the Physician-in-Chief of the Knight Cancer Institute at OHSU. Dr. Rocha’s clinical practice encompasses all aspects of benign and malignant disease of the liver, bile ducts and pancreas. He has been funded by ASCO and the Cholangiocarcinoma Foundation. Dr. Rocha currently sits on the editorial boards of HPB, Annals of Surgical Oncology, Journal of Surgical Oncology and PLoS ONE. He has served on the NCI Pancreas Task Force, ASCO Gastrointestinal Cancers Advisory Council and ASTRO Liver Cancer Guidelines Committee. He is currently the surgical lead of the International Cholangiocarcinoma Research Network (ICRN) and Vice-Chair of the Southwest Oncology Group (SWOG) Surgery Committee.
